Establishing the cause comes first
When did the change start, does it affect one or both sides, and is it painful? These details help guide the examination. Medication, supplements and anabolic steroids also belong in the discussion. Further investigations depend on the findings. Do not stop or start medication or hormones on your own.
When to seek prompt assessment
A new hard lump, rapid growth on one side, skin dimpling or nipple discharge should be assessed promptly by a doctor. These signs do not automatically mean cancer. They do, however, need assessment before any cosmetic decision.

Interpreting the evidence
Adult men completed SF-36 before and six months after gland removal. The study concerns gynecomastia, not every enlargement caused only by fat.

Observation or treatment?
Not every case of gynecomastia requires surgery. Its course, cause and symptoms determine whether observation, treatment of an underlying condition or an operation may be appropriate. Adolescents and adults have different needs. Individual assessment should not be replaced by booking a procedure from photographs.
What can surgery change?
The technique depends on glandular tissue, fat and excess skin. Liposuction and gland removal serve different purposes and may be combined. Nipple position and existing differences between the two sides also inform planning. See male breast reduction for details.
Training and natural proportions
Training changes muscle and can improve general fitness. It cannot specifically exercise away enlarged glandular tissue. Our aesthetic reference is your own body shape. An extremely flat chest or a fitness photograph is not a universal treatment goal. Understanding the male chest explains the different tissues.
Discussing risks and recovery
Before deciding, discuss bleeding, infection, scarring, altered sensation and possible contour differences, among other risks. Also clarify the surgical location, support at home, compression, follow-up and the demands of your sport. Personal clearance is more useful than a fixed training break for everyone.
